Edition control reminder

When you reference exposure limits or control requirements, cite the exact adopted regulation/standard and edition. Don’t paste numeric tables into uncontrolled documents.

1) Laser inventory template (minimum viable)

FieldValue
System name / ID
Location
Owner (PI / manager)
LSO / EHS reviewer
Wavelength(s) present
Modes / operating statesnormal / alignment / service/bypass
Max credible output by state(power or pulse energy + rep rate)
Class marking (if applicable)
Beam paths and terminations(attach sketch)
Safeguardsenclosures / interlocks / shutters / indicators
Eyewear requirements by state(wavelength band + OD)

2) Scenario table template (operating-state discipline)

3) Alignment SOP template (minimum baseline)

Template use note: Keep this as a working SOP shell. Replace each template field with system-specific details before authorization or use.

Scope: Template field: laser or system ID, alignment operating state only
Authorized roles: Template field: alignment-authorized names and role
Required eyewear: Template field: wavelength band(s) and OD by operating state

Pre-job checklist

Alignment steps (example structure)

  1. Start at lowest feasible hazard state (power reduced, shutter control).
  2. Move one optic at a time; verify termination after each change.
  3. Keep eyes out of the beam plane (use approved viewing aids/targets).
  4. Stop-work trigger: any uncertainty about beam path, termination, or state.

Post-job restore (mandatory)

Deep-dive: X. Alignment hazards

4) Interlock bypass permit template (service discipline)

Template use note: Treat each permit as a controlled record. Fill every field before work starts, then complete restore verification before closure.

System ID: Template field: system ID
Safeguard bypassed: Template field: door, panel, or other safeguard identifier
Reason: Template field: commissioning, service, or troubleshooting reason
Start time / end time: Template field: authorized time-bounded window
Authorized by: Template field: LSO, EHS, or other competent approver
Temporary controls in place: Template field: barriers, signage, controlled access, and supervision controls
PPE required: Template field: eyewear specification and any additional PPE

Restore verification (must be completed before return to normal)

Deep-dive: R. Engineering controls

5) Periodic audit checklist (behavior-focused)

Template use note: Use this checklist with a defined review cadence and evidence trail, not as an informal walkthrough.

Audit period: Template field: monthly, quarterly, or site-defined interval
Audit lead/team: Template field: assigned role or team
Audit record reference: Template field: record ID or repository path

7) Eyewear issue + labeling log template

8) Incident report skeleton (facts-first)

Date/time:
Location/system:
Operating state: normal / alignment / service/bypass
Exposure pathway: direct / specular / diffuse / unknown
Safeguards status: enclosures/interlocks/shutter/PPE
Immediate response: stop work + medical eval per policy
Preserved facts: beam path sketch, optics state, photos, instrument logs

Contributing factors (system-focused)

  • scenario not covered in SOP / SOP not followed / SOP not realistic
  • engineering safeguards missing / bypass routine / restore verification absent
  • training/authorization mismatch (alignment by non-authorized user)
  • eyewear mismatch (band/OD/state) or usability issue
  • change-control failure (new optics/power not reassessed)

Corrective actions (hierarchy of controls)

  1. engineering changes
  2. procedure/training changes
  3. audit and monitoring changes

Close-out: owner + due date + verification method.

Deep-dive: Z. Risk assessment methodology
